It is up to you whether you choose to create a new treatment plan for every patient visit, or you simply make amendments to one plan across appointments.

There is no black-and-white approach when it comes to treatment plans and how to write them. However, it is important to keep some key tips and tricks in mind when approaching treatment plans. For example, you should maintain an objective stance at all times, as other healthcare practitioners may view the document in addition to the client. It is also important that you avoid abbreviations to minimize confusion, and that any mistakes made are annotated with your signature and date, especially if you choose to stick with using one treatment plan. Treatment plans can be written electronically, or they can be printed out and handwritten, so long as they are stored within a secure database upon completion.

While treatment plans follow a standardized format, it is super important that you work towards personalizing and individualizing treatment plans according to each client you visit. Health issues can present very differently across clients, as we're sure you know, and so it's important to take into account a range of factors unique to the client. You should integrate subjective and objective observational information, in addition to their background and goals, to ensure that their treatment is specific to their situation and experiences.
